New Depression Project

• Patients that completed PHQ-9 at initial visit and at follow-up visit
• Patients who have a PHQ-9 score of > or = to 10 at Index Contact
• Patients who show a 50% or more decrease in the PHQ-9 score at 6 months (+-30 days)
• Patients who have a PHQ-9 score <5 at 6 months (+-30 days)
• Patients that followed up as they were directed, OR if no documentation, returned every three months

Ongoing Optimal Diabetes project

• Patients who had hemoglobin A1c(HbA1c) tested
• Patients who had HbA1c level <=7
• Patients who had eye exam (retinal) performed
• Patients who had LDL-C screening performed
• Patients who had LDL-C controlled (LDL<100 mg/gl)
• Patients who have been screened in the past year for microalbuminuria (excluding those with known nephropathy)
• Patients who had blood pressure reading performed
• Patients who had blood pressure reading <130/80
• Patients on aspirin therapy if age >40
• Patients with documented tobacco-free
• Patients up-to-date on all measures
